This first version of the Clinical Indication Directory was compiled by
medical student Mubeen Yousuf (Queen Mary Medical School London) at Highfield
Surgery, Hazlemere in April 2005. It was based on all repeat prescriptions
that were produced by the surgery during the months of May and June 2005.
If patients do not wish to have indications added to their drugs the phrase
'patient preference no indication' is inserted. Clinical Indications were
designed to help health staff and patients get the best from their medicines
thus warning messages are also included. This version 02/02/06.
